Business Forms

These forms NEED to be filled out within 2 hours of purchasing an item. Fauilure to do so will lead to

• Graphic Business Form
• Youtube Business Form

Without one of these filled out we can not continue. You Must create one for every item purchased, even if its a bundle deal. For example if you purchase the Youtube Intro & Outro deal you will need to create 2 forms!
